I was made to feel at home here. Except when I first entered and was told not to get up, nothing to drink and a diaper put on me when I wasn't incontinent. She wouldn't even tell me her name, however everyone I encountered were wonderful and caring. Especially a tech named Monica. She is the best employee they have, kind. Caring, going above and beyond. She's the best. I'd highly recommend this rehab.Lack of Transparency and Support from Business and Social Services Department. Although the rehabilitation services were strong and the front desk and clinical staff demonstrated kindness and professionalism, my experience with Heritage Park’s Business Office and Social Services was deeply troubling. What should have been a transparent and supportive process for my father instead became marked by inconsistent communication, conflicting information, and a concerning lack of coordination between departments. I was initially informed by the social worker that my father required memory care and that this facility was not an appropriate long-term placement for him. However, when a nurse arrived to assess him for a memory care facility, the Business Manager was overheard providing information to the assessment nurse that appeared intended to influence the assessment in a way that would prevent him from being approved for the very services we were told he needed. This contradiction raised serious concerns regarding transparency, integrity, and the motivations behind these decisions. As someone who visited daily, I witnessed a noticeable decline in my father’s ADLs, which further confirmed that this environment was not suited to his needs.The staff and the physical therapy team were very attentive and conscientious to my needs. They worked with me to set goals and milestones for my recovery and I would not have been able to achieve those goals, had it not been for their support and hard work. The facility was spotless and the nursing staff and aides were the best. I strongly recommend this facility should you need rehabilitation care.I've been visiting my friend for a couple of years here. It looks like a nice clean place. Staff friendly and compassionate. Everyone at Heritage Park is very kind and caring. My uncle has been at HP for about 2 years. Every single caregiver and administrator and receptionist has been nothing less than very kind and helpful the whole time. Caregiving is hard- caregiving for a non- family member has to be the hardest job ever but they always were so gentle and patient with him. Special place in heaven for his caregivers there. This facility's MDS Nurse is next level. I love the spirit of their therapy team. I really could feel the unity among their facility leaders. The building was clean. The receptionist was very welcoming and courteous to every single guest. I'm so glad Mrs. Miller introduced me to this wonderful facility. I was quite pleased with the rehabilitation care received. Found the facility clean, food surprisingly tasty and numerous options. PT and OT a Godsend....to my recovery. Last, Director Kim and entire staff was readily available for and met all my needs.I was told that they do not have enough staff to care for all the patients and that is why I had to wait for my medication. Also, the response time to my call bell was very slow. Overall, I am very dissatisfied with the care I received. The workers are low paid and and have no respect for the residents living there. Her belongings were constantly lost and I personally had to go to their lost and found to find her clothing and bedding on several occasions. I was constantly told her vitals were normal as was her temperature. By Monday she suffered from complete dementia/delirium due to extreme infection that went unnoticed by the nursing staff. My father had a terrible experience at this facility. My Mother in Law was transferred there from Blake Hospital. They were nasty and told a Woman that could not move her arms to "feed" herself". She stopped eating and drinking and was in terrible pain. She was taken to Blake Hospital again.
